Application for Persons with Disabilities Status

Do you have difficulty returning and picking up library materials on time due to a disability? You can apply to change your library card to Persons with Disabilities Status by filling out this form.

Eligibility

Persons with Disabilities Status is for people with disabilities who cannot return library materials on time or pick up holds on time because of a disability, as defined in the Accessibility for Ontarians with Disabilities Act (AODA, 2005).

A valid full-service Toronto Public Library card is required. To get a library card, visit Your Library Card for more information or ask branch staff. Staff can then apply Persons with Disability Status to your account.

Persons with Disabilities Status Application Form

A valid Toronto Public Library card is required to apply online.

Please visit a library branch with your name and address identification. Library staff can then register your new card for Persons with Disability Status.

Library staff will process your application and confirm your status within two business days.
